MINUTES
July 1, 2014

Present: Ginny Bracket (Chair), Betsy Poulin, Sally Butler,
Doug Phillips, Don Robbins, and Robert T. Folsom Sr.
(CEO/LPI)
Absent: Marianne Hubert

1. Meeting started at 7:00 PM.

2. Doug Phillips made a motion to approve the minutes of the June 3, 2014 Vassalboro Planning Board Meeting. A second was made by Sally Butler. Vote: All in favor.

3. Raymond Breton applied for a Major Site Plan Review for a 4,400 square foot addition to the existing Mid-Maine Internal Medicine Building (Map #23 Lot #45).

Don Robbins made a motion to review the Mid-Maine Internal Medicine Building as a Major Site Plan Review. Betsy Poulin seconded the motion. Vote: All in favor.

Ginny Brackett went through the Performance Standards to check to see if the applicant had submitted all required submittals.

Robert T. Folsom, Sr. (Code Enforcement Officer) advised that the project was not in the Shoreland Zone, the Flood Plain or in the Aquifer Protection Area.

Don Robbins moved that the application is complete. Doug Phillips seconded the motion. Vote: All in favor.

The Code Officer, Robert T. Folsom, Sr. requested that the Planning Board require the plans to be certified that they meet the IBC 2009 Code and also that they submit a Commcheck Form to meet the Energy Codes.

Performance Standards on the Mid-Maine Internal Medicine Building Major Site Plan Review were voted on as follows:

Performance Standard #1:
Doug Phillips made a motion that Performance Standard #1 has been met.
Motion was seconded by Sally Butler. Vote: All in favor.

Performance Standard #2:
Doug Phillips made a motion that Performance Standard #2 has been met. Betsy Poulin
seconded motion. Vote: All in favor.

Performance Standard #3:
Doug Phillips made a motion that Performance Standard #3 has been met. Don
Robbins seconded the motion. Vote: All in favor. Note: Chain link fence to be added
on the Lang Street side square of corner and addition of cedar trees.

Performance Standard #4:
Doug Phillips made a motion that Performance Standard #4 has been met. Don Robbins
seconded the motion. Vote: All in favor.

Performance Standard #5:
Don Robbins made a motion that Performance Standard # 5 has been met. Sally Butler seconded motion. Vote: All in favor.

Performance Standards #6:
Don Robbins made the motion that Performance Standard #6 has been met. Motion seconded by Doug Phillips. Vote: All in favor.

Performance Standard #7:
Don Robbins made a motion that Performance Standard #7 has been met. Motion
was seconded by Betsy Poulin. Vote: All in favor.

Performance Standard #8:
Don Robbins made a motion that Performance Standard #8 has been met. Motion was
seconded by Sally Butler. Vote: All in favor.

Performance Standard # 9:
Doug Phillips made a motion that Performance Standard #9 has been met. Motion
was seconded by Don Robbins. Vote: All in favor.

Performance Standard #10:
Don Robbins made a motion that Performance Standard #10 has been met. Motion
seconded by Betsy Poulin. Vote: All in favor.

Doug Phillips made a motion to approve the application for a 4,400 addition to the Mid-Maine Medicine Building as presented. Don Robbins seconded motion. Vote: All in favor.

4. Doug Phillips made a motion to adjourn. Don Robbins seconded the motion. Vote:
All in favor. Meeting adjourned at 8:45 P.M.
